Retainer Practice
Annual Fee Schedule:
$1000 Individual (infant to 39)
$1800 Individual (40 and over)
$3200 Adult couple (over 40)
$2000 Single parent family
(one adult + unmarried dependent children under 18)$3600 Family
(two adults + unmarried dependent children under 18)*dependent denotes that the child is living in your home or is in college up to the age of 18
Insurance:
We will file insurance for BCBS, United, Aetna, and others. However, if you choose to be a member of the retainer practice we will not file claims with these carriers because many of the services and office visits are included within your membership fee. If a procedure or service is not covered by the retainer practice, and is covered through your carrier, we will file it on your behalf. We expect payment in full at the time of service. We also accept Medicaid and Medicare. We are not currently providers for the CHIP's program, but are actively trying to add it!
